So that we are able to give you an estimated breakdown of your dental insurance benefits at the time of your appointment, please be prepared to give us your Insurance Company name, date of birth, policy, group and telephone numbers. This information is generally found on your insurance card.
Upon your arrival…
When you step off of the elevator at the second floor, you immediately enter our reception area. Our Patient Director is there to greet you and gather information for treatment, insurance submission and billing needs. For your convenience and to simplify this process, our registration form and medical / dental questionnaire can be downloaded from this site and completed prior to your appointment. If you choose to complete these forms on the day of your appointment, please plan to arrive 15 minutes earlier than your scheduled time of arrival. It is our mission to make your first visit with us a comfortable and pleasant experience.
